Base recurring schedule: Not listed. Base monthly hours: Not listed hours for 4-week billing month. Base monthly amount paid to Task Teens: Not listed. Any hours requested beyond the monthly hours listed in this addendum must be approved by Task Teens and will be billed separately by invoice. Example: If a month such as July includes five service weeks, the regular two-day weekly schedule may create additional visits beyond the 12 monthly hours included in the base monthly fee. Those additional hours are not included in the base monthly fee and will be invoiced separately.
